Workouts not 'verified' despite confirming calibration and logging via USB flash drive

Whilst the vast majority of my workouts have been recorded correctly as 'verified', I just noted that the past few weeks' worth have not.
I'm not sure why that should be the case as I always use a USB flash drive and always confirm calibration before each session (and recalibrate whenever required).
The BikeErg itself is only six months old and in close to daily use. I have noticed a few issues with the PM5 from time to time, in that it might throw up an error or two and sometimes appears to lose the connection with the flywheel. This has never occurred during a session (only when checking the damper setting for eg, which I also do before each workout) and is usually fixable by removing and reinserting the flash drive and/or turning off/on the PM5. I'd experienced similar issues with the BikeErgs at the rowing club so presumed this was just software bugginess rather than anything else.
All the cables (that I can see/check are secure) and the machine in kept in pristine condition, wiped down and dried after every session.
Any thoughts?
