My heels lift slightly when my shins are horizontal at the catch, but as I begin the drive I push from the balls of my feet. I presume I should work to push from the entire foot but for some reason it's not natural. Perhaps from 20 years of cycling? Am I correct that using the front of the foot alone does not generate as much power? Any recommendations for set-up or technique adjustments to rectify the problem?

A bit of heel lift is not a problem, but you should push those heels back down quickly to start the drive. It is more like a squat in that you want to get those heels involved.

Not everyone can sit on his heels, curl the tongue, play different rhythms in the right and left hands, or wiggle ears. It's genetic.
